Opening a Life Rewards account is as easy as 1-2-3!

  1. Download and review a Summary of the Plan Handbook.

    I have reviewed the Handbook Summary and I am eligible to participate in Life Rewards
     
  2. Download and read the Offering Circular or the Prospectus for the fund(s) you would like to invest in.
     
  3. Download and complete the Life Rewards Enrollment Forms. Once these forms are complete, mail them to Church Extension Plan at:
